An important part of your initial appointment at St. John- Clark Pain Treatment Center is an in-depth evaluation by your Neurosomatic Therapist. We want a complete picture of both your symptoms and your story. Our intake form is very thorough, and we recommend setting aside 15 minutes to complete prior to your visit.
